Buying from Independent Contractors

An ICQ is required when payments are being made to individuals for services rendered. The ICQ or Independent Contractor Questionnaire determines whether or not an employer/employee relationship exists between the individual providing the service and the University.

To determine if a supplier should be considered an Independent Contractor or not, complete the questionnaire below.

Once complete, submit this to: icqadmin@mcmaster.ca

McMaster University has developed a template Contract for Services. This has been reviewed by the legal department and provides a general contract for service arrangements with a supplier.
